    Enter the user’s web address by adding name after /. The user’s address is located on each OneLife iD as www.onelifeid.com/johnsmith or onelifeid.com/johnsmith . All you need to do is enter johnsmith. Note NOT case sensitive but no spaces please.

    4-Digit PIN

    Enter PIN found on each OneLife iD. PIN located on back of ID band tags or printed on Dog Tags, cards and stickers. Normally 4 digits eg 2325. Note no spaces please.

    Our Club Member iD Tag is a great way for clubs or organisations to enable their members to carry their emergency, medical and contact information, demonstrate membership and show their team colours.

    The professionally printed PVC Tags in club colours and design can be personalised with the members choice of contact, emergency or medical information.

    Just select your club or organisation from the list below and add your personal details:

    • 4 lines of personalised information including:
      • Name
      • Contact details
      • ICE details including medical conditions or emergency contacts
    • Web address for your online ID profile

    Supplied as a set of 3 MiniTags. Each MiniTag Size (one third credit card) can be worn or attached to clothing and kit.

4 steps to create your custom iD Tag:

  1. Select your Tag design
  2. Add ID information and emergency contacts
  3. Choose a name for your online profile
  4. Preview your Tag and select quantity

3. Online Profile Name

Choose a name for your unique online profile. Please use 5 – 12 letters or numbers with no spaces. Find out more about online profile name Help
